Cancellation Policy

CANCELATION POLICY:

We are deeply grateful for the trust and confidence our patients place in us by making us are part of their healing.

We strive to take of each individual as we would our own family, while maximizing our availability for everyone.

We understand that life moves rapidly.  Cancellation requests made more than 48 hours prior to the appointment will not incur any fees, as we’ll be able to help a patient on our wait list who’s asked to see the doctor sooner.

Cancellations made within 48 hours will incur fees as follows:

New Appointments:  $250

Follow-up appointments:  50%

IVs and procedures:  $50

Reminders go out 72 and 48 hours in advance of your appointment to help avoid any fees.  We appreciate your support in helping us help our entire ‘family’!

Thank you,

Insurance Coverage

Case Integrative Health is out of network for consultations.  This allows us to extend visit times and create the connections and treatment plans that lead to long-term health, vs. band-aid level approaches afforded to doctors with 5-10 min. visits.

If you upload your insurance information to our site, we can help submit your consultations and non-covered treatments for out-of-network reimbursement per your plan's policy.

Lab work, prescriptions and insurance-coverage pain procedures are submitted to insurance automatically.

What's the Fastest way to connect with my Provider?

If you need to connect with your provider (Doctor, PA, RN, etc), please send your question through the Patient Portal Questionnaire link.  That is the ONLY HIPPA-Compliant way we can respond with medical advice.

Standard turnaround time for PPQs is 3-5 days.  If the matter is more urgent than that, we recommend calling the Front Desk so we can triage the message for the provider to respond faster.

Reminder:  Case Integrative Health is not an emergency or urgent care facility.  If you have a medical emergency, please dial 911 or go to the ER.  If you have an urgent issue, please visit Urgent care.
